Hospice Nurse, RN - Hospice - Roanoke based

Job Summary



Be a part of a dynamic, growing hospice team that is dedicated to providing exceptional care at end of life. This position primarily consists of conducting hospice consults with patients/families and completing hospice admissions. Some after hours on-call required. Will work closely with hospice clinical team lead.

 

This position will serve as a member of the Hospice team and serve as a case manager for assigned hospice patients. Will coordinate symptom management for assigned patients.  Coordinates care of patient/family unit in collaboration with members of hospice IDT team, Medical Director and attending physician.

Minimum Qualifications


Education: Must be a graduate of an approved school of professional nursing/RN program.


Experience: Must have a mimimum of one year experience in hospice, home health, oncology, or medical surgical nursing. Previous hospice/home health expereince preferred. 


Licensure/Certification: Must have a current valid Virginia RN license.  Must have valid driver's license, automobile insurance and personal vehicle for making home visits.


Other: Must have a clear criminal records check as approved through the Virginia State Police criminal record exchange and found to be  free from barrier crimes.
