Provides psych-social support to Palliative Care patients through the Palliative Care services consult team. Applies knowledge of psycho-social care in support of hospital quality improvement initiatives.   Responsible for psychosocial assessment of patient and families; referrals to outside counseling agencies as appropriate; making recommendations to staff nurses and physicians on the plan of care, and assistance with obtaining information to help support patients and families with serious or life-limiting illnesses. Works with nurses, physicians, and other disciplines to ensure patient’s goals are being followed.  Provides counseling for patients and families as needed.  Provides education on palliative care and end-of-life issues to staff, physicians, patients and families.  Documents an assessment and makes recommendations per protocol. Coordinates and oversees the bereavement program with the help of clerical support.  Supports Carilion’s hallmarks of service excellence and quality for all job responsibilities.

Minimum Qualifications

Education: Master’s degree in family therapy, counseling, social work or related discipline required.

 

Experience: Provides support and/or supervision, appropriate to licensure and scope, to masters and undergraduate level interns and other administrative functions which may include new staff orientation and training, coordination of ongoing Palliative Care Specialist education.

 

Licensure, certification, and/or registration: Licensed Clinical Social Worker (LCSW);  OR MSW with expectation of obtaining ACHP-SW within 2 years of employment; OR Licensed Professional Counselor (LPC).

 

Other Minimum Qualifications: Must successfully complete orientation and competency validation for position.  Demonstrates critical thinking and creative problem solving skills to move patients effectively and efficiently through the continuum of care.  Ability and willingness to work as an integral member of a multi skilled team.
