Care Coordinator-LPN- Home Care

Full Time position. Home Care services department.

Provides care management for specific patient populations, utilizing clinical expertise, communication and problem- solving skills to achieve optimal clinical and resource outcomes. Promotes cost-effective care by minimizing fragmentation, maximizing coordination and facilitating patient movement through home care. Applies protocols when appropriate and facilitates referrals, providing linkages from referral sources to Home Care Clinicians. Promotes interdisciplinary collaboration and teamwork to progress the plan of care.
 

• Clinical review of referrals for accuracy and completeness
• Review of all referrals to ensure unacceptable ICD-10 diagnoses are flagged and acceptable diagnoses are requested from the referral source.
• Review of all referrals to determine alignment of Face-to-Face with referral documentation. 
• Follow up with referral sources to request and receive matching Face-to-Face with referral documentation.
• Review of orders that are part of referrals to ensure that they are specific and correct.
• Follow up with referral sources to request and receive orders which are specific and correct whether in writing or over the phone (verbal order).
• Review to ensure that the referring physician and following physician are accurate and associated with the appropriate documentation to meet regulatory compliance. 
• All clinical review questions/supervision will be addressed with the RN Quality and Patient Safety Manager.    
• Coordinates care and services.
• Performs patient/family data collection to assist in identifying individualized care management needs. 
• Effectively communicates with clerical and clinical support staff to coordinate patient care needs, ensuring that all admission criteria are met.
• Coordinates obtaining referral information and works with home health liaisons to appropriately integrate the patient into the health care continuum. 
• Monitors work queues to ensure all referral processes are followed within appropriate time frames while utilizing resources effectively.
• Facilitates physician documentation of data that accurately reflects the patient's condition, co-morbidities, treatment and procedures that support admission to home health while remaining in compliance with all procedural and regulatory aspects.
• Coordinates and manages all referrals and admissions with a focus on comprehensive, accurate, and effective communication.
• Assist patient with scheduling F2F appointments with their PCP as needed.
• Communicates all operational/workflow questions or concerns to the Business Operations Manager promptly.

What We Require:

Education: Graduate of an accredited school of practical nursing required.

Experience: Five years recent experience in home health or related field required​​​​​​​.

Licensure, certification, and/or registration: Current licensure as a Licensed Practical Nurse (LPN) in the State of Virginia required.  ICD-10 Coding certification required within one year of hire​​​​​​​.

Other Minimum Qualifications:  Must demonstrate knowledge and competency in the following areas: positive interpersonal oral communication skills; effective written communication skills; analytic skills; team player; courteous; ability to resolve complaints/problems; customer-focused philosophy of service delivery; ability; willingness to work as an integral member of a multi-skilled team. Also demonstrate knowledge and competency in: computer literacy; community and system resources; effective interpersonal relations; analysis and research methods.
