This is an opportunity to be a part of the amazing Emergency Department (ED) Observation Unit at Carilion Roanoke Memorial Hospital (CRMH) in Roanoke, VA. This position is a collaborative role working closely with ED Observation (ED Obs) and geriatric providers and staff. Must have good communication skills and be highly motivated as this is an autonomous position. This role will be part of the Geriatric ED implementation and in doing so, advocacy for our geriatric patients and families is imperative. You will aid in connecting patients with follow-up care to include primary care and specialty follow-up care. Will work as a resource for nursing staff. Will assess patient for potential barriers to discharge, identifying needs and potential follow-up needs. Will collaborate with other support services to ensure successful discharge, i.e. Social Work and Case Manager. You will be responsible for data collection based on unit specific goals, disseminating data to provide feedback to providers and nursing as appropriate. Team cohesiveness is important with this role, and being part of celebrating wins for each member of the unit along with global unit wins is also an important part of this position.
This position is based in our groundbreaking Emergency Department Observation Unit. This department, while still embedded in our Emergency Department is still separated with detail to more In-depth diagnostic testing and continued observation for patients who may not meet criteria for inpatient admission but require further medical management than outpatient settings can provide in a timely manner. As part of the ED this unit is fast paced, like the ED with the goal of timely discharge with full support network in place for successful outcomes. The ED Obs unit is also a close-knit unit that functions as a team from morning rounds to discharge.

What We Require:
Education: Graduate of an accredited School of Nursing required. Bachelor’s degree preferred.
Experience: Minimum of 3 years RN experience required. At least 2 years experience in relevant specialty or department, as defined by the department, desired (ex: hospital setting, vascular, rehab, long term care, home health, cardiology, ambulatory, surgery).
Licensure, certification, and/or registration: Current VA RN license required. Must successfully complete orientation and competency requirement. Must obtain and maintain appropriate certifications as defined by the department.
- BLS-HCP required within 1 year of hire or at time of hire
- ACLS, Conscious Sedation, CCRN, ONC, Pain Management and other modality specific certifications (ex: Cardiology, Surgery, Orthopedics, Vascular) certifications desired.
Valid Virginia, or state specific, drivers license required for employees working in Home Health or departments where the work requires traveling.
Other Minimum Qualifications: Effective interpersonal, communication, and team-working skills required. Strong critical thinking, conflict resolution, organization and effective judgement abilities required. Must successfully complete orientation and competency validation requirements. Must be able to meet essential job functions. Designated areas may require Knowledge of Medicare/Medicaid and other third party payor regulations. Proficiency in Cover My Meds, Medicaid navigation preferred. Knowledge in Utilization Management and patient billing is preferred. Certification in Geriatric nursing preferred.
