How You’ll Help Transform Healthcare:
Follows documented physician/licensed independent provider orders, within the boundaries of the Nurse Practice Act and established standards, policies, and practice guidelines. Organizes and plans work with input from the patient/family with specific outcomes and demonstrated use of sound clinical judgment and resources. All day-to-day operations of the role are handled independently with minimal assistance. Advice and guidance are sought from the Director on an as needed situational basis and when there is a major impact on activity outside the unit. - The Transplant Coordinator delivers and demonstrates strong clinical experiences in patient care by accurately documenting direct, age-specific care to a patients or groups of patients based on medical orders and an integrated, multidisciplinary plan of care. This position manifests a high level of expertise in the assessment, coordination and care of complex patients, families, or communities to actual or potential health problems.
- The position demonstrates a high level of expertise in the assessment, diagnosis, and treatment of the complex patients by safely administering prescribed medications, identifying incompatibilities and/or interactions and their responses to actual or potential health problems. The position assists the health care team with the prevention of illness and injury, maintenance of wellness, and provision of comfort. The position also provides direction, education, and leadership.
What We Require:
Education: BSN preferred or will consider minimum of five years in similar role.
Experience:
Experience within a surgical department preferred, and a strong history of patient navigation and coordination of care for surgical patients.
Licensure, certification, and/or registration: Active Virginia RN license.
Life Support: BLS, ACLS
Other Minimum Qualifications: Prolonged standing and walking in the hospital and OR setting, as well as holding clinics with patients in an outpatient setting. Able to lift, position or transfer patients. Able to lift supplies and equipment.
