Job Summary

Provides personal care and comfort measures to patients of all ages in their homes in an efficient and organized manner as directed by the registered nurse. Communicates effectively with the case manager, Clinical Team Leader and/or other members of interdisciplinary team.

The individual must be able to demonstrate an understanding of personal care needs across the age continuum and the need to notify an RN for any questions or concerns. The individual must be able to provide care as needed and as described in departmental policies and procedures.  HHA spends largest volume of time standing while performing patient care and sitting while driving and documenting.  Physical demands include lifting, turning, transferring patients who may be in excess of 200lbs.  Other physical demands include carrying equipment needed to perform patient care.  Frequent exposure to extremes in temperatures dependent on individual home environments as well as seasonal weather changes.  Frequent travel in adverse weather conditions such as rain, snow, sleet, wind, high/low humidity, etc.  Must be able to operate a motor vehicle in order to perform patient care.  Potential exposure to blood and body fluids as well as other chemicals, i.e.

Minimum Qualifications

Education: Satisfactory completion of Geriatric Nursing Assistant, Certified Nursing Assistant or Home Health Aide course that meets State, Federal, and CMS requirements for Home Health Aide training or is deemed competent by the home health agency as required by CMS regulations.

Experience: One (1) year experience in a healthcare setting preferred.

Licensure/Certification: Must have valid driver’s license.  Successfully completes HHA Competence Program as established by home health agency. Annually will complete by return demonstration all of the appropriate skills on the Home Health Aide competency checklist, and acquires twelve (12) hours inservice per year and receive a yearly evaluation of “meets standards” or better. AHA BLS-HCP must be secured by end of supervised orientation and maintained.

Other: Effective interpersonal, communication, and teamwork skills required. Must successfully complete orientation and competency validation for position. Must have reliable transportation.  Clear criminal record check.
