Job Summary

This position is located at Carilion New River Valley Medical Center (CNRVMC) in Christiansburg, VA.  CURRENT VIRGINIA RN LICENSE REQUIRED


Facilitates daily coordination of patient care.  Interacts with staff, patients, families, visitors, members of the health care team and external partners  to develop, coordinate, and manage the plan of patient care.   Demonstrate  knowledge of the principles of growth and development, and possess the ability to collect and assess data of patient populations.    Must work autonomously  and in a team environment.  Utilize delegation and critical thinking skills to provide quality service through a team oriented,  collaborative approach to patient care.   Demonstrate  the role of patient advocate, protector of patient’s rights, provider of psychological/social/cultural support, and  tolerate stressful situations.  Maintain current knowledge and compliance with accrediting agencies (Joint Commission) within the health care environment, and  state and federal regulations.  Performs work in a cost-effective manner keeping cognizant of patient/family needs and the needs of the organization.  

May be exposed to infectious diseases, bio-hazards, and hazardous materials, including regulated medical waste and combative patients.  Regular travel for role specific education and  coordination of services may be required.





This position will support Carilion’s hallmarks of service excellence and quality for all job responsibilities. The coordinator uses systems of care processes in decision-making, patient assessment, development and implementation of care coordination methods and systems to improve the overall quality and efficiency of patient care for the designated department and population served.  Will provide direct and indirect patient care as needed; oversees the delivery of patient care; and provides day to day management of programs to coordinate patient care as designated by the department.  Coordinates the delivery of safe, high quality patient care; identifies areas for study; and participates in and/or supports research. Works in compliance with all State, Federal, and other regulatory requirements. Serves as a resource and mentor to staff and team members and demonstrates excellence in interprofessionalism.

Minimum Qualifications

Education: Bachelor's degree required in a human services, counseling, social work, psychiatric rehabilitation, psychology, sociology or vocational rehabilitation as needed to satisfy the current DMAS and licensing standards per the Board of Health Professions which at this time requires one year of clinical experience with substance abusing population.

Experience: Minimum of 1 year substance abuse clinical experience in providing direct services to persons with a diagnosis of mental illness or substance abuse; or one year of clinical RN experieince required.

Licensure, certification, and/or registration: Current Virginia RN license required,  An RN who is also a  Certified Substance Abuse Counselor (CSAC) or a CSAC-Assistant under supervision prefered BSN Preferred. De-escalation training must be successfully completed within one year of hire and then maintained going forward. BLS-HCP required within 1 year of hire or at time of hire.

Other Minimum Qualifications: Effective interpersonal, communication, and team-working skills required. Strong critical thinking, conflict resolution, organization and effective judgement abilities required. Must successfully complete orientation and competency validation requirements. Must be able to meet essential job functions. Designated areas may require Knowledge of Medicare/Medicaid and other third party payor regulations.
