Job Summary

Hospice- Full Time- New River Valley


Ensures implementation of Hospice Bereavement Program Services. Provides information and support to patients and families who experience loss and grief in our community. Provides supervision to Hospice Volunteers, students, and other Hospice team members who participate in or facilitate bereavement activities.
 

  • Organizes, initiates, and directs sequential bereavement contacts with Hospice families that are experiencing normal grief reactions. Contacts consist of a combination of in person visits, telephone calls and written contacts. Follows clients for 13 months after patient death. 
  • Identifies patients and families at risk for complicated and compound grief reactions and makes appropriate referrals within the community for those clients. 
  • Organizes and facilitates Pathways Bereavement Support Groups. Trains coworkers and/or volunteers to assist with the groups. 
  • Works collaboratively with other Interdisciplinary team members to provide comprehensive Hospice Services.  Attends Interdisciplinary Team meetings. Presents assessments and updates of clients.  
  • Takes leadership role in preparations for Camp Treehouse, Annual Memorial Service and other programs as requested by Manager.
  • Attends or ensures staff attendance at patient funerals/wakes/memorial services. 
  • 75% of patient funeral events should be attended by this employee or other staff member.
  • Demonstrates excellent verbal and written communication skills.  Documents Bereavement Services in electronic medical record in compliance with multiple regulations. Practices confidentiality at all times. 
  • Works with the Volunteer Coordinator to recruit, train, and retain volunteers to assist with Bereavement activities. 
  • Serves as a resource regarding grief management for the hospital and the community. Participates as member of Hospice Speaker’s Bureau and gives community presentations as requested by Manager. 

Minimum Qualifications


Education: Must have Bachelor’s Degree. 

Experience: Must have two years experience in working with terminally ill patients and/or bereaved clients. Should be able to demonstrate computer proficiency with office suite software.

Licensure/certification: Must have valid Virginia Driver’s license, use of an automobile with appropriate automobile insurance in order to make home visits/funeral visits as required.

Other Minimum Qualifications: Must pass Virginia State Police Criminal Records Exchange Criminal Records Check as processed by Carilion Human Resources. Must demonstrate willingness to be available for patient/staff/community crises. Should live within local access for phone/pager network.
