Carilion Clinic Senior Director - Care Management

Responsible for the system-wide leadership of care management at Carilion Clinic.  Maintains overall strategic and operational responsibility for case management, social work, utilization management, and centralized resource center.  Aligning care management in the system to support the goals and objectives of the Carilion Clinic.   Responsibilities include: Strategic goal and action planning, leading and building teams, leadership development of the organization's care management leaders, cross departmental collaboration with population health strategies and acute care outcomes management, evidenced based design supported with analytic/data. Effectively represents Carilion Clinic within the healthcare community and to the public.
 

Accountable for the design, implementation, management and measurement of results of care management strategies and programs.  Plans the implementation of directives, standard policies and procedures and protocols as approved by service line vice president, senior management or the board of directors.

 
Responsible for alignment of all strategic case management initiatives across the system.  Organizes and develops strategies and processes to support the transition of patients into the post-acute environment  Responsible for the alignment of care management practices by developing relationships with system stakeholders.  Dyad partnership with Carilion Clinic's Internal Physician Advisor Medical Direction.
 

Partners with key stakeholders (including medicine, nursing, outpatient case management and post-acute care) to develop and execute operational plans for managing care transitions and transition outcomes.  Manages multiple, complex and potentially large departments, as well as designated administrative and support staff to assure cost effectiveness and achievement of goals and budgets.  Monitors performance quality and performance metrics while also providing recommendations on operational design and efficiencies.

Minimum Qualifications

Education:  Master’s Degree required.   If the area(s) of responsibility employ and have responsibility for supervising RNs, a Master's degree is required and a nursing degree at the minimum of the BSN level is required for Magnet compliance pending the area is within the Magnet scope. Requires an RN or MSW with at least 5 years of direct case management experience.

 

Experience:  5-7years experience in management, preferably health care environment, demonstrating increased and/or diversified responsibility in a system wide framework.

 

Licensure:  Licensure and/or certification in medical or management related discipline.  RN and/or Certified Case Manager preferred.

 

Other Minimum Qualifications:  Advanced leadership, planning and organizational skills to manage multiple and potentially large, complex departments and to direct the successful completion of initiatives and achievement of fiscal goals.  Demonstrated interpersonal and communication abilities to support and promote a respectful, inclusive culture of service excellence.  Proven skills in clinical, operational and human resource management, including staffing, planning, performance management, team work and process improvement.  Statistical analysis and fiscal management skills
