The Clinical Coordinator is a vital member of the interdisciplinary healthcare team responsible for supporting quality patient outcomes through evidence-based care coordination and frontline clinical engagement. This role focuses on early identification, response, and documentation related to high-risk diagnoses such as sepsis and stroke, while also participating in quality initiatives, education, and data reporting across care settings.
- Collaborate with physicians, nurses, and other healthcare professionals to support evidence-based care delivery and improve patient outcomes.
- Provides clinical oversight and direction to frontline staff in identifying and managing patients with conditions such as sepsis and stroke.
- Participates in patient care activities for high-acuity cases, including interdisciplinary rounds (IDRs), rapid response events, and care planning.
- Monitors Emergency Department and Inpatient units for signs of clinical deterioration and escalates concerns appropriately.
- Responds to clinical alerts and assists with timely and accurate documentation related to patient status and care provided.
- Educates staff on best practices, quality standards, and clinical protocols to enhance disease recognition, documentation, and management.
- Contributes to quality improvement initiatives by identifying performance gaps, supporting interventions, and tracking progress.
- Collects and disseminates data related to clinical quality measures and assists in outcome monitoring.
- Serves as a clinical resource across the enterprise for best practices related to disease-specific care and patient safety.
- Provides direct care support during emergent patient scenarios as needed to ensure optimal outcomes.
- Supports occurrence reporting and helps identify potential patient safety issues, participating in follow-up and resolution efforts.
- Maintains current knowledge of clinical and regulatory standards related to quality and safety.

What We Require:
Education: Graduate of an accredited School of Nursing required. Bachelor’s degree Preferred.
Experience: 3 years of RN experience required. 2 years of experience in hospital setting, ED or ICU specialties preferred.
Licensure, certification, and/or registration: Current VA RN license required. Must successfully complete orientation and competency requirement. AHA BLS and ACLS required. May be required to obtain/maintain PALS and NRP.
Other Minimum Qualifications: Demonstrates excellent problem solving, interpersonal, communication, priority setting and work competency skills. Promotes a respectful, inclusive culture of service excellence.
Excellent problem-solving skills and ability to exercise independent judgment. Strong verbal and written communication skills with ability to effectively interact with all levels of management, internal departments and external agencies. Working knowledge of various computer software applications.
Must be detailed oriented, able to synthesize complex data and utilizes sound clinical judgement based on years of leadership and clinical experience.
Savvy with crucial conversations. Intuitive and autonomous, anticipates nursing practice needs.
