The Inpatient Coder is responsible for concurrent or retrospective coding of inpatient accounts, using ICD-10 CM coding guidelines. Requires daily consultation with physicians to ensure accurate capture of the principal diagnoses and procedure, and Present on Admission Indicators to ensure proper APRDRG, APDRG or DRG assignment for reimbursement and clinical reporting. Acts as a documentation improvement consultant with the physicians, and assists them with correct documentation for coding and reporting clarity. Interacts with Case Management, QM, UM, Infection Control, and Pharmacy by generating codes that identify patients with potential long LOS, JCAHO Core Indicators, Nosocomial or Community-acquired infections and adverse drug reactions, promoting real-time intervention and improved quality of patient care. Masters the Severity of Illness and Risk of Mortality indicators and consults with physicians to ensure documentation reflects these characteristics in our patient population. Organization-wide Inpatient Volume is approximately 50,000 - 55,000 per year.
Primary job duties of the Coder-Inpatient include, but are not limited to, the following:
- Must be skilled in the application of ICD-10-CM and CPT coding methodologies for coding of Inpatient records and well as ED, Ambulatory Surgery, and medical necessity for Ourpatient Diagnostic visits.
- Captures the correct principal diagnosis, co-existing conditions, and principal procedure for each inpatient admission. Consults with the physicians to clarify or improve documentation for correct coding assignment to ensure correct data reporting and reimbursement and to maintain compliance with Federal and State regulations.
- Responsible for sequencing codes that a) capture accurate Severity of Illness/Risk of Mortality, to reflect the nature of Carilion's patient population for comparison to other facilities and health organizations nationally, impacting quality of care reporting of Carilion's facilities and pay for performance initiatives.
- Masters all grouper systems used by payers to assign the correct, DRG, APDRG, or APRDRG and maintains a working knowledge of payer requirements for use of HCPCS S, C, G, and D code application.
Minimum Qualifications
Education: High School education required: College degree preferred. Medical Terminology and Physiology & Anatomy required.
Experience: Requires minimum of 2-2.5 yrs previous experience in Inpatient Coding with a 95% accuracy rate . Requires knowledge of body systems, disease processes, anatomy and physiology and medical terminology. Basic computer skills required.
Licensure, certification, and/or registration: RHIT, RHIA, CCA, CCS, CPC, or CPC-H, certifications required for consideration for position.
