Job Summary

Supports Carilion's hallmarks of service excellence and quality for all job responsibilities. Responsible for abstracting clinical information from the hybrid medical record, verifying information concurrently and upon discharge and analyzing records for deficiencies.  Enters deficiencies and abstracted data elements into hospital EMR.  Researches and notifies physicians regarding their  incomplete record status.

Duties and Responsibilities may include by not limited: 

Analyzes each patient record for deficiencies in compliance with medical staff bylaws and regulatory guidelines and enters deficiencies into hospital EMR.  
Verifies and abstracts clinical information from the hybrid medical record and enters in hospital EMR.
Reviews records to assure accurate completion of record deficiencies according to regulatory requirements and updates the record management system.  Responsible for all edits/verifications of the record until the record is completed.  Assists physicians with completing record deficiencies.  
Researches physician delinquent status and notifies physicians of delinquency/suspension status.  Assists with the reinstatement and notification of privileges as needed.  
Maintains working knowledge of hospital specific patient information system and departmental software to facilitate workflow.
Produces key reports as identified by the HIM department for reporting and informational purposes.  
Collaborates with the Supervisor/ Manager to resolve concerns and troubleshoot issues; performs other duties as required to meet departmental objectives to include meeting quality and production standards, attending educational activities in compliance with Carilion and HIM policies, and cross-training in other functions of the department as required.

Minimum Qualifications

Education: High school education required.  Additional courses in medical terminology and anatomy/physiology preferred and working knowledge of basic computer functions.  Associate degree preferred

Experience: One to two years pervious medical records experience preferred.  EMR, PC, Windows and keyboard proficiency required. 

Other Minimum Qualifications: Good communicator, well organized, detail oriented, and good navigational skills between multiple computer systems.  Must have communication skills. Ability to work well with others in a team environment.   Ability to multitask in a fast paced office environment.  Ability to accept/give constructive feedback.
