Hospice Nurse - Roanoke

Job Summary



Will serve as a member of the Hospice team and serves as a case manager for assigned patients. Coordinates symptom management for assigned patients. Coordinates care of patient/family unit in collaboration with members of hospice IDT team, Medical Director and attending physician. In addition will serve as a hospice admission nurse requiring the ability to multi-task, prioritize, and evaluate patient/caregiver needs. Will make visits in patients' homes, hospitals, nursing homes, and adult care centers. Will cover rotational evening/night on-call.  Also performs the duties of lead staff which include but may not be limited to implementing and managing daily activities of the office, maintaining regulatory compliance, reviewing documentation, monitoring of face-to-face documents, re-certifications, participating in performance improvement and other quality improvement initiatives. Is directly supervised by Hospice Manager.

Minimum Qualifications


Education: Must be a graduate of an approved school of professional nursing/RN program.


Experience: Must have a mimimum of one year experience in hospice, home health,  oncology, or medical surgical nursing.


Licensure/Certification: Must have a current valid Virginia RN license.  Must have valid driver's license, automobile insurance and personal vehicle for making home visits.


Other: Must have a clear criminal records check as approved through the Virginia State Police criminal record exchange and found to be  free from barrier crimes.
