Job Summary

Full time Nurse Practitioner - Ventricular Assist Device (VAD) Coordinator -  individual collaborates with patients and the multidisciplinary team to formulate and carry out a plan of care for patients being evaluated for or post ventricular assist device implant in the inpatient and outpatient settings in collaboration with the Advanced Heart Failure and cardiothoracic surgery teams. The VAD NP performs physical exams, collects, documents, and analyzes clinical data, conducts, and interprets diagnostic and therapeutic procedures, orders and schedules laboratory studies and professional consultations, prescribes appropriate interventions and medications, coordinates the care and discharge of patients, provides direct patient care services, and provides education to staff, patients, significant others, and the community.  

The VAD NP conducts rounds and assesses patients with VADS and Heart Failure, including performing history and physical exams, writing notes and orders, performs pre-VAD insertion evaluations, and collaborates with establishing plan of care with multidisciplinary team.
• The VAD NP plans and arranges transitions of care across the health care spectrum and orders, monitors, and interprets inpatient and outpatient tests and diagnostic procedure results to improve patient outcomes and prevent complications and readmissions.
• The VAD NP will care for patients in the inpatient and outpatient settings with responsibilities including office visits, outpatient laboratory monitoring and medication titration, follow-up phone calls, and triage.
• The VAD NP provides expert resource re: multiple VAD systems to include basic care, parameters, and troubleshooting for patients and staff.
• The NP provides training/education to patients (pre- and post-implant and in evaluation phases), caregivers, and clinicians on VAD systems, including alarm management, troubleshooting, and management of VAD-related issues.
• The VAD NP will oversee CRMH's participating in the INTERMACS database.  
• The VAD NP will be an active participant in quality improvement initiatives to improve outcomes using evidence-based care.

No experience required; however, preference give to applicants with previous employment in a cardiac area such as cardiothoracic or coronary care intensive care unit, a cardiac step-down unit or progressive care unit, the cardiac catheterization/electrophysiology lab or a heart failure unit/dept.

Minimum Qualifications

Education: Graduate of an Accredited Nurse Practitioner program.

Licensure/Certification: Current Virginia State Licensed Nurse Practitioner (NP) and DEA license required, or must be eligible to obtain. AHA BLS-HCP certification required. Advanced Life Support certification required. Eligible for prescriptive authority in accordance with State regulations and as authorized by the Virginia Boards of Medicine. Capable of meeting credentialing or job function requirements as needed based on the requirements of the practice and of managed care payors and insurers. Qualifies for malpractice insurance and is accepted by Carilion Clinic's malpractice insurance carrier.

Other: Able to work collaboratively with physicians and other health care providers. Demonstrates good written and oral communication skills. Computer literacy required for automated computer systems, including electronic medical records.
