Responsibilities, but not limited to:

 

  • Demonstrates understanding of Hospice concept of care and the complex issues related to caring for the terminally ill.  Provides competent, compassionate care in a variety of settings. 
  • Assesses, plans, implements, and evaluates patient care to achieve optimum patient outcomes.  Promotes safety during vulnerable periods of transition and upholds a culture of patient centered care. 
  • Age specific admission and ongoing assessments are completed, documented and used to develop a plan of care.
  • Collaborates with members of the hospice team/patient/family to develop, implement, and evaluate an age-specific interdisciplinary plan of care.  Revises plan according to changes in patient status.
  • Responsible for managing all aspects of patient care.  Case conferences on assigned patient during regularly scheduled interdisciplinary meetings. 
  • Treatments and medications for patients of all ages are accurately administered and documented utilizing automated computer systems, including electronic medical records. 
  • Ensures patient care orders are current, accurate, implemented, and evaluated for effectiveness per providers' orders. 
  • Patient/family (caregiver) educational needs are identified in a timely manner, incorporated into the plan of care, implemented, and documented appropriately. 
  • Participates in performance improvement activities.  Maintains awareness of outcomes/scorecard measures - works towards positive results.  Assists with providing evidence-based educational activities. 
  • Consistently demonstrates the role of patient advocate, protector of patients' rights, and providers of psychological/social/cultural support.
  • Exhibits clinical expertise in rapidly changing environment.  

Plans strategies to prevent avoidable re-hospitalizations, reduce length of stay, turn-around time and resource consumption, and improve patient outcomes.

What We Require:

Education:  Graduate of school of professional nursing.

Experience:  One (1) year experience in hospice, home health, oncology, or medical surgical nursing preferred 

Licensure, certification, and/or registration:  Currently licensed as Registered Nurse in the Commonwealth of Virginia or new graduate of approved program eligible to sit for the Virginia licensure exam.  For Home Health, must have valid Virginia Drivers License and pass a Criminal Record Check as per Virginia state law. Must obtain /maintain AHA BLS-HCP.                                                  

Other Minimum  Qualifications:  Must successfully complete orientation and competency requirements.  Keyboarding skills required for automated computer systems, including electronic medical records.  Must be able to meet essential job functions.
